Medical College, Srinagar. w.e.f 24-10-2016 to 23-11-2016 from 10 am to 1.00 pm on all working days.
- Date of Birth Certificate.
- 8th Pass Certificate
- Permanent Resident Certificate.
- Sweeper Community Certificate to be issued by concerned Tehsildar.
- Category certificate, if any.
- An affidavit duly signed by Ist Class Magistrate to the effect that their qualification is 8th and not beyond.
