The candidates are advised to enclose the attested true copies of the following documents with their application forms which should reach the office of the undersigned by or before 31/3/2023.

Attempt Certificate of all examinations of MBBS/MD/DNB/Ph.D. of any other qualification issued by the University.

Distinction /Honour /Medal/Academic Merit Certificate, if any

Post-Graduate Degree/Diploma Certificate.

Professional Publications in an indexed National as per MCI standard Medical Journals.

Experience Certificate wherever required.

Date of Birth Certificate.

Domicile/ Non-Domicile may also apply subject to the condition that preference will be given to the Domicile candidate.

Any other certificate pertaining to the Speciality.

Affidavit duly attested from Ist class Magistrate stating that the doctor is not in active Govt Services
