J&K Govt Orders Inquiry into Maternal Death at SKIMS MCH Bemina

Committee Headed by GMC Srinagar Principal to Probe Circumstances, Fix Responsibility for Lapses if Any; Report Sought Within 15 Days

The Jammu and Kashmir Government on Tuesday constituted a three-member inquiry committee to investigate the circumstances leading to the death of a patient at SKIMS Medical College Hospital (MCH), Bemina (JVC Hospital), directing the panel to examine the case in detail and submit its report within 15 days.

According to Government Order No. 585-JK(HME) of 2026 dated July 28, 2026, a copy of which lies with news agency , the inquiry has been ordered into the death of a patient that occurred on April 26, 2026, at SKIMS MCH Bemina.

The inquiry committee will be headed by the Principal, Government Medical College (GMC), Srinagar, with the Head of Department, Gynaecology & Obstetrics, GMC Srinagar, and the Medical Superintendent, Children’s Hospital Bemina, as members.

The committee has been tasked with examining all medical records, treatment sheets, monitoring charts, nursing notes, investigation reports, operative notes, consent forms and other relevant documents related to the case.

As per the Terms of Reference (ToR), the panel will assess whether postpartum haemorrhage or any other obstetric emergency was adequately considered when the patient repeatedly complained of worsening symptoms, examine whether post-delivery monitoring was conducted according to established medical protocols, and determine whether the patient was shifted prematurely from the delivery area.

The committee will also investigate whether retained placental tissue, delivery-related trauma, uterine abnormalities or clotting disorders contributed to the bleeding, ascertain reasons for any delay in diagnosing haemorrhage, and evaluate whether such delays adversely affected the patient’s outcome.

It has further been directed to review the timeliness and adequacy of surgical intervention, examine the role and conduct of all doctors, nursing staff and other personnel involved in the patient’s treatment, fix responsibility for any negligence, omission, misdiagnosis or deviation from accepted medical standards, and recommend corrective measures to prevent recurrence of such incidents.

The Government has directed the Principal, SKIMS Medical College Hospital Bemina, and all concerned officers and officials to extend full cooperation and provide all relevant records to the inquiry committee.

The committee has been asked to submit its detailed fact-finding report, supported by documentary evidence, within 15 days from the issuance of the order.
